Natural Resources Police corporal Sloan Cooper is returning from arresting three men who attacked hikers in western Maryland when she walks into a convenience-store robbery. The robber shoots her in the forehead and chest. Her partner, Joel Warren, provides lifesaving emergency care, and she is revived three times during surgery. Sloan recalls an apparent out-of-body experience, including seeing Joel outside the operating room. She survives, but her chest injuries, weakness, and recurring nightmares require extensive rehabilitation. Her boyfriend Matias ends their relationship by text. Under medical restrictions, Sloan moves back to her parents' home near Mirror Lake in Heron's Rest, where Elsie, Dean, her sister Drea, and their extended family support her recovery.

As Sloan struggles through short walks and physical therapy, another story develops nearby. Clara, a hospital nurse, and Sam, who works at a nursing home, are secretly selecting people who have been clinically dead and resuscitated. Clara believes medical intervention has improperly reversed God's will. She and Sam bring selected people to their house, question them about what they recall from the period of unconsciousness, and kill them. Their religious rationale is their own interpretation, not a fact confirmed by the narrative. Janet Anderson disappears just before Thanksgiving after a previous drowning and resuscitation; additional victims include Wayne Carson, Celia Russell, Arthur Rigsby, Zach Tarrington, Lori Preston, and Alyce Otterman. The disappearances are initially handled by different agencies in Maryland, Pennsylvania, and West Virginia.

Two brothers from New York, Nash and Theo Littlefield, buy and begin restoring the former Parker house. Nash has left investment management to become a licensed contractor, while Theo turns away from an expected legal career to help establish the Fix-It Brothers. Theo begins dating Drea. Sloan, meanwhile, recovers enough to drive and return to service, receives departmental psychological clearance, and applies for a sergeant vacancy in Captain Travis Hamm's Western Region unit. She buys a bungalow near her family, passes the promotion examinations, and becomes a sergeant in January. She meets Nash in person when she commissions a bathroom renovation, and their business relationship develops into a romance. Their contrasting family backgrounds become important as Nash describes the controlling parents from whom he protected Theo. Theo and Drea eventually become engaged and purchase a home of their own.

Unable to forget Janet Anderson, Sloan searches publicly available reports for comparable disappearances. Information from former classmate Diane Blakley brings Zach Tarrington's case to her attention. She consults Detective Frank O'Hara in Uniontown and interviews witness Adam Rusk, who recalls a woman and a light-colored van near the hotel where Zach vanished. During a conversation with Nash about finding what the missing people had in common before they disappeared, Sloan realizes that Janet, Zach, and Arthur had all previously been resuscitated. She subsequently identifies other possible cases. O'Hara and a multi-state task force investigate the connection, with Sloan receiving information as an approved consultant rather than a formal task-force member. She theorizes that a closely bonded pair may regard their actions as a religious duty; these remain hypotheses, and she does not yet know the perpetrators' identities.

During inquiries at hospitals connected to the disappearances, Sloan unknowingly speaks to Clara at an emergency department. Clara accesses Sloan's records and learns that Sloan herself had been revived during surgery. She comes to believe Sloan is a witch interfering with their mission and begins watching her. Sam shares that belief. They identify local chef Terrance Brown, who survived a lightning strike, as another target. Terry is engaged to Hallie Reeder, a friend of Sloan's. Clara and Sam observe Sloan's home and movements, then intercept Terry on his way home from work. His colleague Boone Hastings finds his abandoned car, and O'Hara briefs Sloan. She reports seeing a suspicious couple near Terry's restaurant and elsewhere in town, and a police artist prepares a drawing of the man. The suspects' connection to the case has not yet been established by investigators.

Clara and Sam decide to hold Terry while attempting to take Sloan as well. Sam enters Sloan's house while she is absent. When Sloan returns, small changes in the room tell her someone has been inside. Clara arrives in a dark blue van, pretending to need directions, but Sloan recognizes her from the hospital. Sam moves against her with a weapon, and Sloan fires in self-defense, fatally wounding him. She subdues and restrains Clara. Nash calls emergency services and assists Sloan as she provides investigators with information to locate Terry. He is rescued alive and taken to hospital. O'Hara identifies Clara as Clara Burch and gives Sloan the opportunity to conduct the formal arrest, including charges connected to the seven earlier victims and Terry's disappearance. The scene establishes Sam's death, Clara's arrest, and Terry's survival, not a completed prosecution or conviction.

In the Epilogue, Sloan tells Nash that investigators have recovered evidence of the earlier crimes, including recordings and personal effects. Although she acted in self-defense, she is shaken by the shooting and knows a formal review of her use of force will follow. Nash supports her and asks her to move into his house, where he has already considered making an office for her. She agrees. They discuss his intention to propose after Drea and Theo's wedding and her preference for an outdoor wedding in May or early June. They are not yet formally engaged. The ending connects Sloan's recovery and second chance at life with her professional choices, the resolution of the immediate threat, and a future she and Nash intend to build together.
